It is known as 4 different titles mainly due to different covers:  DANZIG LIVE, DEATH NOTICES, LIVE IN SAN FRANCISCO or THE FELT FORUM.  All of the wax is Danzig Records: DR 1000 and pressed in 1989 with 3 tracks first side and 2 tracks, second side.  There are a total of 500 orange, red or yellow vinyl copies with 3 different sleeves or no sleeve at all.  500 total in all regardless what the sleeves say.  James and Kirk on track 5 supply additional vocals.  Taken from 1/5/89 at the Stone in San Francisco.
